Hydrogen Ignitors, Conclusion of 10 CFR 21 Investigation See Letter of January 28, 1983 i
Gentlemen:
Under the requirements of 10 CFR Part 21, we are offering concluding remarks abut a possible reportable defect relating to the model 6043 hydrogen ignitors. On January 28, 1983 Power Systems, a Morrison-Knudsen Division, informed the Commission and other interested parties including customers in writing of a potential product failure in nuclear safety related apparatus.
Suspect product was returned to our facility and a vendor manufacturing step absence was corrected irrespective of whether the product exhibited a leak.
All revised products were tested. The remedial measures included:
1) restoring a brazing ring to strengthen a crimped bond on a tubular glow plug element against twist failure that might dcVelop when assembling the tip element to its face plate mount and 2) testing all corrected product for
. leaks after assembly.
t l
l Torque............................. 16+.1 Ft-lbs.
S ta ti c P res su re.................... 70T 2 PSI G.
Duration........................... 10 minutes minimum.
None exhibited leaks.
Replacement face plate assemblies with brazed glow plugs were exchanged satisfactorily at the River Bend Nuclear Station on February 8 and 9,1983.
Entire ignitor assemblies were returned from site storage at Perry Nuclear Station and revised prior to their return by March 25, 1983.
These proceedings and other technical matters were provided to the glow plug g
manufacturer who has recognized the need to provide better sealing. We have informed each of our clients affected by the possible defect and will now inform them of its successful conclusion.
