This letter and its Attachment provide the report called for in Item 4 of subject Bulletin. The information given in the Attachment pertains to Unit No. 2 of the Donald C. Cook Nuclear Plant. It is planned to perform a similar inspection, as required, for Unit No. 1 during the upcoming 1983 refueling outage.

Attachment to AEP:NRC:0703A This Attachment provides the report requested in Item 4 of IE Bulletin No. 82-02 for Unit No. 2 of the Cook Plant.

Response to Item 4.a:
Procedures for the items applicable to IE Bulletin 82-02 were reviewed for adequacy. These procedures were updated, as deemed necessary, to fulfill the requirements of the Bulletin. A generic crocedure, 12 MEP 5021.001.009. " Maintenance Procedure for Bolted Closures and Flanges" was developed as a supplement, if needed, to the above mentioned procedures.
In regards to the use of approved chemicals to minimize the possibility of SCC, PMI-2160 delineates the chemical requirements of any substances used in primary or safety related systems. A list of approved chemicals and cleaning agents is available for use at the plant site.
To the best of our knowledge and understanding of the requirements of Item 1, Item 1 is complete.
Response to Item 4.b:
During the recent refueling outage, the following bolted closures were examined as having been identified to be the ones falling under the scope of Item 2 of the Bulletin:
1)
Reactor Vessel Head Closure Studs 2)
Pressurizer Manway Bolts 3)
Steam Generators No. 22 and No. 23 Primary Manway Bolts.
Response to Item 4.c:
The following are the details on the results of the bolt inspections:
1)
Reactor Vessel Head Closure Studs NDE was performed on Reactor Vessel Studs #1-#10, #12-#39,
- 40-A, and #41-#54. The NDE included visual inspection and magnetic particle inspection as required by Action Item 2.
All NDE inspection results were found to be acceptable.

Reactor Vessel Stud #11 could not be removed from the Reactor Vessel flange. A UT-60 examination was performed in place, in lieu of VT and Mr examinations. Inspection results were acceptable.
Reactor Vessel Stud #2 examination revealed the presence of indications. The indications were removed by filing and the results were found to be acceptable.
2)
Pressurizer Manway Studs NDE.was performed on the 16 Pressurizer Manway Bolts. The NDE included visual inspection and magnetic particle inspection as required by Action Item 2.
All NDE inspection results were found to be acceptable.
3)
Steam Generator Primary Manway Bolts _
3a) Steam Generator No. 22 NDE was performed on the Primary Manway Bolts of Steam Generator No. 22. The NDE included VT and MI inspections as required by Action Item 2.
One bolt was rejected by VT for partial loss of threads. This bolt was discarded and replaced with a new bolt.
The threads were chased on all bolts prior to installation. Final NDE inspection results were acceptable.
3b) Steam Generator No. 23 NDE was performed on the Primary Manway Bolts of Steam Generator No. 23.
The NDE include VT and h'r inspections as required by Action Item 2.

acceptable.
