Enclosed are three copics of revised NRC Form 366 which is a part of the following Davis-Besse Unit 1 Licensee Event Reports:
81-004,81-073, 82-064, and 83-008. These forms have been revised to change the informa-tion in block 24 NPRD-4 Form Submitted.
There has been no change to the supplemental information sheets included with each Licensee Event Report. Therefore, please replace only the Licensee Event Report NRC Form 366 in each of the above mentioned reports with the attached revisions, retaining the respective supplemental informa-tion sheets.
