ATTACIIMENT I VIOLATION Technical Specification 6.8(a) states, " Written procedures and adminis-trative policies shall be established, implemented and maintained that meet or exceed the requirements and recommendations of Section 5, Facility Administra-tive Policies and Procedures, of ANSI 18.7-1972 and Appendix A of Regulatory Guide 1.33, November 1982."
Radiological and Environmental Services Procedure No. RE-HPP-2,1, Rev. 2,
" Radiation Exposure Authorizations", developed pursuant to the above, requires the Radiation Exposure Authorization (REA) be posted at the control point sign-in area.
Contrary to the above, on January 14, 1983, REA No. 2356, " Repair and/or Replace Ac Necessary #32 SFP Cooling Pump", was not posted at the control point sign-in area used by contractor personnel.
RESPONSE
It is felt that this was an isolated incident and is not generic in nature. The SFP cooling pump repair was being controlled and accessed from our primary control point; the Quality Assurance inspector, a contractor, entered from the contractor's control point, where that REA would not normally be maintained. The control point Health Physics technician verified that the REA was active, but failed to obtain a copy for the individual's review.
Corrective action taken was to instruct all control point personnel of the need to assure that an active REA copy is present at that location prior to issuing a REA sign-in sheet.
RES Procedure No. RE-HPI-2.25, " Instructions to Access Control Watch Personnel", will be revised to incorporate specific requirements for issuance of REA sign-in sheets.
In addition, Procedure No. RE-HPP-2.1, " Radiation Exposure Authorizations", will be revised to require centrol point watch personnel to sign each REA sign-in sheet, signifying that he has verified that a copy of the active REA is present prior to its issuance.
The aforementioned corrective action will be implemented on or before April 1, 1983.
