1. For reportaole incidents for hazardous waterials as cefined in C.1:
a. The person discovering the nazardous materi al incident should call extension 211 and report I the incident.

o. Upon notification of a hazarcous material incicent, tne Shift Engineer or his cesignee will take the actions specifisc in Appendix A to General Company Order No. 19, "Cnart for Reporting Pajor Troubl e," (Attachment S).

IMPLEMENTING PROCEDURE FOR FIRE:

FIRE CHIEF (CESIGNATE] SHIFT FOREMAN)

A. Pg3 POSE The purpose of this procedure is to cescrioe the duties E anc respcasibilities of tne Fire Chief in the event of a fire.

C. PREREQUISITES

1. The oesignated Snift Foreman will function as Fire Chief, unless otherwise specified oy tne Snift E Engineer.

D. DRECAUTIONS

1. None.

E. LIMITATIONS AND ACTIONS

1. The Fire Chief has tne responsioility for the coordination of fire fignting efforts ana the ceployment of fire fi gnt ing personnel at tne fire location; the Fire Brigade as well as Fire Companies No. 1 and No. 2 will receive direction frcm the Fire Chief.
2. Upon l ocating ano veri f y ing the fire, tre Fire Cnief will inform the Shift Engineer of the fire's location.

I Tne Shift Engineer will request Security to de-activate tne computer controlled coors tnat provice access to the fire.

3. The Station Fire Marshal snall os notifico immaciately of fires of major proportion, fires invclving large Icss of equipment ano any fire requiring tne Off I Site Fire 00partment. For fires other tnan those

LZP-1340-2 Revision 2 March 15, 1963 above it is at the discretion of the Snift Engineer whether to immeciatel y noti f y the Fire Marsnal or wait until the Fire Marsnal is on site.

In any case, the Fire Marsnal snould os notified as soon as feasicle for fire reporting purposes.

F. PROCEDURE

1. Upon notification of a fire, tne Fire Chief nas tne E following duties and responsibilities:
a. Proceed to the fi re location wi tn prctective clothing and communications equipment.
o. Coordinate preparatory efforts.
c. Assume leadersnip of the Fire Brigade.
d. Direct fire fighting personnel effcrts (actions of personnel, ceployment of equi pment, use of E equipment, methocs to oe used, cirection of fire attack, etc).
e. Evaluate the severity of the situaticn and tne need for additional assistance.
1) If need for the assistance of Fire Company No. 1 is expected, contact tne Snift Engineer wno will aispaten tne requestec manpower to tne aesignatea l ocation.

NOTE l

Once on the scene, Fire Company No. I will function uncer the direction of Fire Officer No. 1 ano reports to the Fire Chief.

2) Request the Off-Site Fire Department (Fire Comp. 02). If the neea for the assistance of Fire Company No. 2 (Off-Site Fire i

Department) is anticipataa:

a) Notify tna Shift Engineer immediately.

f. After the arrival of the Off-Site Fire Department consult with the Ji f- S i te' Fi re Department Senicr Officer regarcing tne oest metnods ano equipment to extinguisn tne fire.
g. Work in conjunction with the Off-Site Fire Department to extinguisn ene fire.
n. Keep Snift Engineer apprised of the neea for adoitional back-up (such as fire-fignting, med i cal , Rad-P r otec t i on , e tc . ) .
i. Perform other auties as necessary to extinguish tne fire and place the fire area in a safe Condition.

I J. Request the Station Fire Marsnal os notified I of the fire and its status.

k. Request guidance, direction or infcreation from the Station Fire Marsnal.
1. Consult with the Station Fire Marshal as to tne best methods, tecnniques, ano equipment ta extinguish the fi ra.
2. Upon issuing tne "All Clear", the Fire Chief will request Security to return the involvec computer controlled cocrs to their pre-emergency status.

3. Ensure all equipment used is returned tc its cesignatec location in working order t. r ol l nose, replace nose, I replace extinguishers, restore systems, etc.). Any missing or damagea equipment sna11 oe reported to the Fire Marshal. Only properl y functicning equipment I shoulo be lef t in the equipment storage lccation.
4. If the Off-Site Fire Cepartment was requireo, provice assistance in the pick-up and removal of tneir B equipment and assist in their timely exit from tne Site.
