DESCRIPTION OF DEFICIENCY Bergen Paterson, a sub-supplier to Applied Engineering Co. (APCO), used magnetic test methods fo r non-des truct ive examinations that were in conflict with those speci fied.

The welded clevises examined for weld acceptability and initially accepted by Bergen Paterson are not ac cep t-able for BVPS-2 application.

AFFECTED COMPONENTS 1

The af fected components are clevises used on pipe rupture res traint s.

PRELIMINARY SAFETY EVALUATION If the pipe rupture res traint s should fail after a pipe bresk, other safety related eq uipment within the vicinity of the ru ptur e could be damaged.

An engineering evaluation of the weld de ficiencies is c ont inuing.

4.

IMMEDIATE ACTION TAKEN, APCO issued a stop-work order to Bergen Paterson on May 5,1983.

The stop-work order has now been lif ted based on an acceptable program for the remanu facture of the c levises.

In order to assure conformance to specification, APCO has assigned a resident inspector to Bergen Pater-son, and DLC is pe rfo rming increased surveil lance at the vendo r 's d

ADDITIONAL REPORTS The results of the evaluation are expected to be available by mid-July.

l An additional report on this issue will be submitted by July 20, 1983.
