Dear Mr. Martin:

1 Your letter of May 23, 1983, forwarded Combined Inspection Report 50-277/83-12 and 50-278/83-12.

Appendix A addresses one item which does not appear to be in full compliance with Nuclear Regulatory Commission requirements.

This item is restated below along with our response.

10 CFR 50, Appendix B, Criterion V, and the Peach Bottom l

Quality Assurance Plan, Vol!1me III, Section 5.0 require that activities affecting quality be accomplished in accordance with documented instructions and procedures.

Contrary to the above, on April 22, 1983, activities affecting quality were not accomplished in accordance with documented instructions and procedures as evidenced in the following examples:

1.

Licensee procedure A-42, Jumper Log Procedure, requires accurate records be maintained of jumper installations, however, an accurate record was not maintained in that jumper number 4-5 (1982) was logged in the Unit 3 log when atually installed in a Unit 2 cabinet and was not cleared from the log when removed from the cabinet.

2.

The licensee Quality Assurance Plan, section 6.2, requires the use of current procedures when performing activities, however a superceded revision of Surveillance Test 12.lA B307150258 830713 PDR ADOCK 05000277 c)

e Mr. T. Martin Page 2 was used when the surveillance was performed on February 20, 1983.

This is a Severity Level V violation (Supplement 1).

RESPONSE

1.

The cause of this occurrence was individual personnel error.

The subject jumper was correctly installed on Unit 2 pending replacement of a defective thermocouple on Unit 2; however, the instrument technician who performed the jumper installation inadvertently logged the jumper in the Unit 3 logbook.

When the thermocouple was replaced the jumper was appropriately removed but, because the jumper had been incorrectly logged, the Unit 3 logbook entry was not closed out at that time.

During later review of the Unit 3 logbook the incorrect log entry was discovered, identified as such, and closed out.

The technician involved has been counselled regarding the need for accuracy and attention to detail in his work.

2.

The issuance of the superceded revision of the Surveillance Test 12.1A was caused by an administrative oversight by the Surveillance Test Coordinator.

The Surveillance Test Coordinator had assembled a package of Surveillance Tests for the then upcoming outage but did not check for procedure revision at the time of issuance.

In the future, the Surveillance Test Coordinator will check to ensure that current revisions of tests are being issued.
