57300, JOB E-5590, FILE NO. 956C,-M-62 This letter constitutes a final report concerning the subject deficiency. initially reported to the Commission on October 29, 1982 under the requirements of 10CFR50.55 (e).
As indicated in the previous report, QA-2280, dated March 7, 1983, fire dampers as manufactured by Air Balance Inc. (ABI) and supplied to the project by Waldinger-Young & Bertke (W-Y&B) are
. located in various safety related HVAC systems in the plant.
The fire damper blade is held in an open posiFinn by "J" hooks joined to a fusible link.
This configuration restrains the damper blade from closing until a temperature of 160 F is reached, the link melts and the damper blade closes and isolates the associated ductwork.

Ventilation System was identified in which the fusible link had i
I melted and the hooks latched onto the damper blade, prohibiting proper closure of the damper.
This problem was attributed to the fact that the hooks pointed upward toward the blades.

a Mr. J.G. Keppler Regional Administrator June 15, 1983 LOZ-83-0050 Page 2 To prevent recurrence of this problem, the orientation of the "J" hooks on all seismic ABI fire dampers have been reversed so that the open end is pointed away from the blade.
W-Y&B performed the work of repositioning the "J" hooks and surveillance reports of their QA/QC inspections are available on site.
We trust the above will be found acceptable as final report under 10CFR50.55 (e).
