Dear Mr. Ilaynes:

NRC INSPECTION REPORT 50-354/82-12 NO. 1 UNIT llOPE CREEK GENERATING STATION Your letter dated November 5, 1982, transmitted the above referenced Inspection Report which contained a Notice of Violation citing two items of noncompliance.

The following response is provided in accordance with the Notice of Viola-tion.

I.

As of September 30, 1982, varglass tie wraps had not been completely replaced with tefzel tie wraps as stated in a 10CFR50.55(e) report dated May 21, 1982, and documented in MCAR #26.

The replacement activity had been final inspected and accepted by QC and MCAR #26 closed out by Bechtel QA.

Corrective Steps Taken and Results Achieved Motor Control Centers (MCCs) 10D251 and 10D261 were re-inspected and Nonconformance Reports No. 1835 and 1837 issued l

to document the varglass tie wraps found in sections of the MCCs containing operating equipment.

The varglass tie wraps were replaced with tefzel tie wraps and the MCCs were cleaned to remove the cut varglass wraps and strands.

Corrective Steps Taken to Preclude Recurrence Responsible construction personnel have been instructed on the importance of a complete and thorough final inspection for all Class IE installations.

The responsible Quality control Engineers have been instructed in the inspection methodology to be used in situations where thousands of small items must be located / replaced.

12/1/82 Additionally, Bechtel QA has instituted a verification sampling program to preclude the reporting of inaccurate information on NRC items of concern.

Date of Full Compliance The date for full compliance for the MCCs was October 4, 1982.

On October 25, 1982, PSE&G QA concurred with the first verifi-cation plan submitted by Dechtel QA.

Main Steam Expansion Joint 1-XJN-1B, in that a weld request I

had not been initiated to identify to QC and Field Welding Engineering that welding was in progress, and a Quality Control Inspection Report had not been issued to control the welding and NDE activities.

Corrective Steps Taken and Results Achieved Upon identification of the problem, work on expansion joint 1-XJN-1B was halted in accordance with PSE&G and Bechtel Stop Work Orders.

An investigation revealed that the Craft Foreman and Superintendent were not completely familiar with the docu-mentation requirements for base metal repairs.

The Craft Fore-man and Superintendent have been instructed on welding documen-tation requirements.

The undocumented welding was identified on Nonconformance Report No. 1836.

Closure of the NCR will be dependent upon NDS results on the subject welding.

In addition, all open Field Change Orders have been reviewed by Bechtel Field Engineering to ensure that the proper documen-tation was generated in accordance with the applicable procedures.

No deficiencies were noted during this review.

Corrective Steps Taken to Preclude Recurrence SWP/P-19 has been revised to require Field Welding Engineering to verify that the applicable QC Inspection Record has been initiated and validated prior to authorizing the issuance of weld filler material.

In addition, Field Welding Engineering has been put on distri-bution for notification of weld cut-outs which are issued by Field Engineering.

This will ensure that Field Wclding Engineering will not continue authorizing the issuance of weld filler material when the QC Inspection Record is closed due to a cut-out.
