Beaver Valley Power Station - Unit No. 2 Docket No. 50-412 USNRC IE Inspection Report No. 50-412/81-09, Revised Response Gentlemen:
In our letter addressed to the attention of Mr.
Richard W.
Starostecki from Mr.
E. J. Woolever, dated March 10, 1982, we provided our response to the item of violation cited in Inspection Report No. 50-412/
81-09.
The purpose of this letter is to inform you of Beaver Valley Power Station - Unit No. 2 (BVPS-2) change in approach to the resolution of this infraction from our original response.
The violation is repeated for clarification as follows:
NRC VIOLATION 10 CFR50, Appendix B,
Criterion V and ANSI N45.2-6 require that quantitctive criteria, such as dimensions and tolerances, shall be appropriate, for determining satisfactory work specified, as performance and quality compliance.
Contrary to the above, the Control Room Ventilation and Air Condi-tioning System were joined together by bolting without speci fied controls for hole sizes, tolerances in the job specification, work instructions, drawings, and/or standards.
This is a Severity Level IV Violation.
REVISED RESPONSE In our original response in Paragraph 1.0 titled " Corrective Action Taken to Date,"
subparagraph 1.1, BVPS-2 indicated that for the duct connection identified, all the nuts and bolts in the duct connection would be removed and the bolt holes' dimensions measured.
If the bolt holes were found not to exceed the specified bolt diameter by 1/16 inch, they would be acceptable and no further action would be required.
If they did exceed the specified bolt hole by 1/16 inch, then suitably sized washers would be provided to accomplish proper bearing surface.
BVSP-2 revision to subparagraph 1.1 is that for the duct connection identified, all nuts and bolts in the duct connection shall be B305270662 830524 PDR ADOCK 05000412 C
United States Nuclear Regulatory Commission Page 2 removed, scrapped, and replaced with new nuts, bolts, and flat washers behind each nut and bolt head.
The use of flat washers ensure proper bearing surface area for the nut and bolt heads.
In subparagraph 1.2, BVPS-2 responded that for all installed 5/16 inch QA Category I duct connections, each bolt will be removed and replaced with a new bolt, nat, and washers.
BVPS-2 revision to subparagraph 1.2 is that all accessible QA Category I duct connec-tions (5/16 inch bolting) installed prior to March 5, 198?, shall be removed, scrapped, and replaced with new nuts, bolts and flat washers behind each nut and bolt head.
In subparagraph 1.3, BVPS-2 responded by indicating that Specifica-tion 2BVPS-935 would be revised to include a statement that for all installed 5/16 inch QA Category I duct connections, each bolt would be removed and replaced with a new bolt, nut, and washers; and that for all future 5/16 inch bolted connection, washers would be required.
Also, that the vendor would be instructed to use 3/8 inch punch.
BVPS-2 revised response to sub paragraph 1.3 is that all accessible bolts, will be removed and replaced with new bolts, nuts, and washers.
Inaccessible QA Category I duct connections (5/16 inch bolting) installed prior to March 5, 1982, shall not need to be repaired based on certified seismic type tests that have been performed as evidence to the structure integrity of the duct flange bolts under full safe-shutdown earthquake loads.
All duct fabrication after March 4, 1982 will utilize punch or drill sizes attaining a tolerance of 1/16 inch greater than the bolt size to be used.
In paragraph 2.0 titled " Corrective Action Taken to Avoid Future Violation," BVPS-2 stated that 2BVS-935 would be revised to reflect the criteria in subparagraph 1.3.
Specification 2BVS-935 has been revised to include the revision to BVPS-2 original response to subpararaph 1.3.
Tolerances have been specified for duct flange fabrication and additionally, flat washers are requir ed for future duct flange installation utilizing 5/16 inch bolting.
The expected completion date for the revised correction action for the duct connections installed prior to March 5, 1982, is January 31, 1984.
