Senior Reactor Operator license.

Since the individual did not have a bachelor's degree, and had not met the experience requirement to have held a Reactor Operator's license and been i

involved in the performance of licensed activities for at least f

one year, the individual was not qualified for a Senior Reactor Operator (SRO) position.

The individual had also not met the Technical Specification requirements to serve in the dual role capacity as Nuclear Shift Supervisor (NSS)/ Shift Technical l

Advisor (STA) since the individual had not obtained either a i

Professional Engineer's License or a bachelor's degree.

l The attached documentation contains personnel information, the disclosure of which would constitute a clearly unwarranted l

invasion of personal privacy of the individual investigated.

Information that would allow identification of the individual has been redacted.

This information has been held in confidence by PSE&G and its distribution has been controlled and limited.

Therefore, in accordance with 10CFR2.790(a) (6), PSE&G requests that the material identified by brackets in Attachment 1 be withheld from public disclosure.

A redacted copy of the investigation results is enclosed as Attachment 2 for release to l

the Public Document Room.

] applied for site access by, in part, completing a Personnel Access Questionnaire.[

routine background check is required. This background check resulted in several j

Company's psychologist include his financial position and his education. As a result of I

these discrepancies his unescorted access was suspended at i1:30 am on March 17,1994.

I A further review of this matter reveals [

] indicated on his Personnel Access l

Questionnaire that he had attended [

, ]but did not graduate. During his l

interview with the Company's psychologist he stated that he did have a degree. A search of[

] personnel file and application indicated he had graduated from((M]in June 1982 as noted on both his employment application, resume and during an interview with the hiring manager. (Appendice D)[

completed 135 credits and was dropped for poor scholarship in December 17,1981. A j

further investigation determined that 192 credits would have been required to graduate with a degree in BSME.

l A fact finding was held on March 18,1994 with[

] at which time he admitted that he did not currently possess a drivers license nor had he ever obtained a degree.

When asked to explain the discrepancy between his Application for Employment and his transcript, he said he had planned to graduate by June of 1982. However, when asked

how he planned to obtain the additional 57 credits required to graduate he indicated some credits were missing and that he was enrolled and attending in 1982. He also admitted that he had never come forward to notify anyone in the Company that he had never obtained his degree.

Since the fact finding, a further review of the record reveals that f

] completed NRC form 398 on July 31,1985 stating he had a BSME. (Appendice E) This form was submitted to the NRC, a violation of 10CFR50.9 (Appendice E)[

] role as an STA required that he have an Engineering Degree or a Professional Engineer's license.

Since[,

] gid not have a degree the Company is in violation ofits Technical Specifications, section 6 " Administrative Controls." These Technical Specifications allow the Company to obtain and maintain its license to operate these facilities. During 1986, l

] qualifications to be a Shift Technical Advisor. (Appendice G) The Company remained in violation of the Technical Specification from 1985 through 1991.

As a result of the two violations (Technical Specifications and 10CFR50.9) the Company "self-reported" these findings to the NRC on Monday, Marca. 21,1994. That same day an

internal investigation headed by the General Manager-QA/ Nuclear Safety Review was initiated.

A second fact finding was held witt[

3 On November 8,1993, as a requirement to regain Unescorted Access on the Island, you completed a Personnel Access Questionnaire. The questionnaire and a resultant background check showed inconsistencies with information you previsously supplied to the Company. Specifically, your employment application indicated that you were to receive a BS Degree fron{

]by June 1982. Subsequently, you indicated on an NRC 398 form that you had obtained such degree. Your previous representations to the Company and to the NRC regarding your degree status were false. Not only did you not receive a degree by the date you first promised - and never informing the Company of same - but later in 1985 you supplied information on the NRC398 form you still had not

[{eceived a degree. On the contrary, you knew all along that you w

~

credits for degree completion.

In addition, your drivers license has been suspended since 1988. From that time, to the present, you have driven Company vehicles without a license subjecting the Company to serious liability and violating state law, as well as Company rules. You have also driven your personal car on Company property during the time your license was suspended.

Again, you knew that you needed a license to fulfill the requirements ofyour job and that you were ot'igated to inform the Company as soon as you lost your license. As a result, fact findings vere held with you on March 18 and 25,1994 to address the Company's concerns You admitted that you did not come forward at any time during your employment to notify the Company you had not obtained a degree or that you lost your driver's license You were hired to fill a position that under the Company's Technical Specifications required an Engineering Degree or a Professional Engineers License. It has been determined that you falsified documents, placing the Company in violation of Technical Specifications, section 6 " Administrative Controls" and 10CFR 50.9 by providing such false information when submitting form 398 to the NRC.
