LER 32-19, Unit 1
Dear Mr. Haynes:
This letter is to confirm our verbal notification of your Mr. Ralph Architzel of the discovery that manual valves in the hydrogen sampling line were closed prohibiting opertion of the hydrogen analyzers.
At 1830 on 4-15-82 with Unit 1 in Mode 1 it was discovered that the manual isolation valves inside containment for the six hydrogen sample points were shut, thus rendering the hydrogen analyzer inoperable (T.S. 3.6.5.1). The valves were immediately opened, terminating the event. The redundant hydrogen analyzer was out of service for modifications relating to TMI Action Plan item !!.F.1.6.
A complete description of the events leading to this discovery and any further corrective action will be addressed in a follow-up report.
