Dear Mr. Starostecki:
This letter responds to your letter of December 23, 1982, which forwarded the report of the routine inspection of activities authorized by NRC License No. CPP R-9 5, conducted by Messrs.
J.
C.
Higgins and P.
Hannes of your office on October 14-November 29, 1982.. Your letter stated that it appeared that one of our activities was not conducted in full compliance with the NRC requirements.
The apparent noncompliance and our response follows:
Apparent Noncompliance with 10CFR50 Appendix B, Criterion V, FSAR Section 17.1.5A, and QA Manual Section 2 10CFR50, Appendix B, Criterion V, the Shoreham FSAR, Section 3 7.1.5A and the Quality Assurance Manual, Section 2.3.1 require activities affecting quality be accomplished in accordance with appropriate instructions.
Electrical installation specification No. SH1-159, page 3-36 limits terminal lug bend to a maximum of sixty degrees.
Page 1-16 requires that equipment be protected from damage be fore,' during, and after installation.
Engineering and Design Coordination Report (E& DCR)
No. F-9732-E requires a minimum terminal stud thread protrusion of at least two threads for torque switch j umpering at the limit switch terminals of motor operated valves.

Staroatscki Page Two January 21, 1983 Contrary to the above, activities affecting quality were not accomplished in accordance with appropriate instructions, in that, on November 18, 1982:
1.
A terminal lug bend for valve E51*MOV-045 was approximately ninety degrees.
2.
The insulation on one electrical lead in the terminal housing of valve E51*MOV-045 was deteriorated with exposed bare wire.
3.
The torque switch jumper at the limit switch terminal of valve Ell *MOV-36A did not have two thread protrusion.
Comments 1.
While reterminating control cables on E51*MOV-045 valve limit switch per Repair Rework Request (RRR) E51-177, the workman inadvertently installed a one Amp lug with the tongue on top of the barrel.
Subsequent tightening caused the excessive bend that was reported.
The deficiency was noted just prior to the scheduled C&IO of the valve.
2.
During retermination of the subject E51*MOV-045 cable per RRR E51-177, the electrician scraped the insulation down to the bare conductor.
This was the condition described as deterioration of the insulation.
Again, the deficiency was noted just prior to the scheduled C&IO.
3.
Two thread protrusion was not achieved on the limit switch terminal of valvo Ell *MOV-36A as required by E&DCR F-9732 October 1977.
- However, the installation was in accordance with good work practices.
Full thread engagement, flush with the top of the nut, was obtained and subsequently determined to be acceptable.
Corrective Actions and Results 1.
During subsequent implementation of C&IO Package E51*29E, the lug was repaired as required.
2.
LILCO Deficiency Report (LDR) No. 954 was issued by the Operational Quality Assurance (OQA) Section to describe the bare conductor condition and to

obtain details for proper corrective action.
It was determined that an engineering disposi-tion was needed so Engineering & Design Coordination Report (E&DCR) No. F42433A was issued to provide engineering direction.
Repair was effected by means of Maintenance Work Request 82-1662.
I 3.
The OQA Sec' tion issued LDR No. 963 to describe the deviation from the two thread protrusion require-ment of E&DCR F-9732, October 19, 1977.
The dis-position was to revise the E&DCR as E&DCR F-9732A which permits and justifies full thread engagement in lieu of the original two thread protrusion.
Actions Taken to Prevent Recurrence 1 & 2.
On December 3, 1982, Startup Support Electricians and Startup Technicians were reinstructed in the proper techniques for installing and terminating control cables in accordance with Specification SH1-159.
3.
No further action required.
Date of Full Compliance All actions were completed by January 12, 1983.
