Perform an inventory of equiprent, publicatiors, and supplies as outlinea on the TSC Ecuipnent, PuDl ications, anc Suppl ies CheCKli st (Attachment A), noting any missing items en the fern.

2.

Any equipment or supplies found tC De missing or in short supply will oe replacec at tne tine of invention or if not availaole will oe ordereo.

Tre cate and initials of the person correcting the ceficiency will De placec in the comment section of tne attacnment.

3.

Return tne compl etea cneckl i st to the HGaltn Pnysics Office for review oy Healtn Physics Supervisicn.
