The following information is submitted in response to I&E Inspection Report 50-321/82-41 concerning the inspection conducted by Mr. B. R. Crowley on December 13-16,-1982:

VIOLATION Technical Specification 6.8.2, as implemented by plant procedure HP-9,

" Procedure Writing, Use and Control", requires approval, issue and control of procedures, and procedure revisions in accordance with plant procedure HP-9.

Contrary to the above, procedure changes were not approved, issued and controlled in accordance with plant procedure HNP-9 in that on December 14, 1982, an unapproved, uncontrolled change to plant procedure HNP-6914 was being used for qualification of class 1 pipe welders.

This is a Severity Level V Violation (Supplement I.E.).

RESPONSE

Admission or denial of alleged violation:

The violation occurred as stateo.

Reason for the violation:

This violation was due to a misunderstanding on the part of the repair agency at the onset of the program for repairing the recirculation piping.

The violation occurred when an unsigned, typed instruction stated that

- qualified welders would use GPC qualification procedure HP-6914, with certain exceptions, without the instruction beir.g approved by the Plant Manager.

As noted in the inspection report, no work was performed by welders under the provisions of this unsigned, typed instruction.

Corrective steps which have been taken and the results achieved:

The personnel involved in the violation were immediately counseled in the correct method of procedure revisions.

The unsigned, typed Instruction was immediately removed from the weld test shop and welders were qualified in accordance with the proper GPC procedures.

Corrective staps which will be taken to avoid future violations; This appears to be an isolated case.

GPC will assure that all partier, concerned in future activities of this nature will be fully knowledgeable in all arms of procedure usage and site quality assurance -

requirements.
