10 CFR 1911(a)(c) requires in part that each licensee shall post current I
copies of Form NRC-3, " Notice of Employees", wherever individuals work in or frequent any portion of a restricted area.
Contrary to the above, on December 16, 1982, it was observed that current copies of Form NRC-3, " Notice of Employees",- were not posted.
This is a Severity Level V Violation (Supplement IV).

Admission or Denial of the Alleged Violation TVA admits the-violation occurred as stated.

Reasons for the Violation if Admitted The latest revision date to NRC Form-3 is June 1982. This revision was-not received at TVA's central office from NRC until mid-August -1982.
At this time, the central office revised Division Procedure ~ Manual' (DPM) N74A6 to incorporate the new NRC Form-3 The DPM was approved and.sent to the plant site in mid-October 1982. At this point, the Sequoyah Nuclear Plant Standard Practice SQA49 was revised and the new NRC Form-3 was in the process of being posted at the time of the routine inspection.
3 Corrective Steps Which Have Been Taken and the Results Achieved The current revision to NRC Form-3 was posted on December 16, 1982.

Corrective Steps Taken to Avoid Further Violatinns TVA's central office will send new revisions to NRC Form-3 immediately to the plant site upon receipt. Also, Sequoyah Nuclear Plant's Compliance Section Instruction Letter No. 11 has been revised to verify once per six months that the current revision to NRC Form-3 has been posted.
