Dear Mr. Haynes:
NRC INSPECTION REPORT 50-354/82-15 NO. 1 UNIT HOPE CREEK GENERATING STATION Your letter dated December 20, 1982, transmitted the above referenced Inspection Report which contained a Notice of Violation citing one item of noncompliance.
The following response is provided in accordance with the Notice of Violation.
As of November 9, 1982, an NRC inspection of initial installation of pipe supports and restraints, performed after inspection by Bechtel QC to QCI P-2.10, identified four installations not in accordance with the requirements of the design documents.
l Subsequent investigation found that one of the four suspect hangers was, in fact, in comoliance with the design requirements.
The Inspection Report stated that for Drawing No. 1-P-BE-047-l H02(Q), Revision 3, the reinforcing "I" beam section of the i
hanger was not coped as required by Detail 1 of the drawing.
Following destructive examination of the hanger, we demonstrated to the NRC Inspector's satisfaction that the "I" beam had been l
l coped as specified by the drawing, and as the QC inspection record had attested.
Therefore, this item should be deleted from the j

The variable support shown on Drawing No.1-P-EG-155-H01(Q),
Revision 2, as installed was incapable of adjustment.

November 10, 1982, Bechtel Quality Control issued Nonconformance Report (NCR) No. 1906 to document and control the resolution of this condition.
Bechtel QC personnel inspected all variable supports of the same design at Elevations 54' and 77'.
The results of the inspection indicate that the condition noted above is an isolated instance.

On the hanger installed per Drawing No. 1-P-BC-0 4 6-H0 5 (Q),
Revision 4, a fillet weld had been used where a partial penetration weld was required by Drawing P-0596(Q), Revision 2.
On November 10, 1982, NCR No. 1908 was issued to document and control the rework of this hanger.
The completed rework was accepted by QC on December 30, 1982.
Bechtel QC reviewed all safety related hanger drawings and identified 230 hangers with similar weld details.
A reinspec-tion of those hangers identified eight additional hangers that did not comply with P-0596 (Q) requirements.
These hangers were identified on NCR No. 1924.
NCR No. 1924 also identified 52 hangers for which conformance could not be verified due to welds wrr ad, painted, or ground flush on the ends.
Similarly, 32 h
3ers were also identified on the NCR as being inaccessible for reinspection of partial penetration welds.
Disposition of these welds will be determined by Bechtel Project Engineering in San Francisco and documented on the NCR.

The hanger installed per Drawing 1-P-EG-155-H02(Q), Revision 0, exhibited an oversized weld condition and distortion to re-straint member.
This was evaluated and found acceptable by Bechtel Project Engineering.
Corrective Steps Taken to Preclude Recurrence 1.
Bechtel Quality Control conducted and documented training sessions for QC personnel on November 10 and 29, 1982, emphasizing hanger detail inspection.
In addition, the fit-up of hangers which require partial penetration welds per Drawing P-0596 (Q) will be inspected by QC prior to welding to assure proper weld geometry.
Bechtel construction super-vision was notified by memorandum on December 2, 1982 of this additional hold point.

Bechtel Project Engineering has instructed their design personnel to verify Drawing P-0596 (Q) weld configuration i

criteria are incorporated whenever a hanger drawing is revised.
By January 19, 1983, Bechtel construction management will is ue a memorandum to the appropriate field engineering and craft supervisory personnel to reinforce the use of Drawing P-0 596 (Q) weld criteria when fabricating and erecting hangers.
Bechtel Quality Control Engineers have been instructed to identify overcize welds in the remarks section of their Inspection Reports and to notify the responsible Field Welding Engineer.
If the oversize weld condition is acceptable, the Field Helding Engineer will document his acceptance by initialling the applicable QCIR W-1.00 in the remarks section.
PSE&G Quality Assurance will instruct Bechtel QC to formally incorporate in their QCI W-1.00 the method to document acceptance of oversize welds by Field Welding Engineers.
Date of Full Compliance The date of full compliance will be January 19, 1983.
