Reference:

(a) Letter from H.J. Miller to Cordell Reed dated February 25, 1991.

(b) Telephone Conversation between B. Burgess (NRC) and R. Rcdtke (CECO) on March 4, 1991.

Mr. Davis:

The referenced letter transmitted the subject inspection report which identified a concern regarding the preparation of an out-of-service for certain Unit i drywell floor drain sump containment isolation valves.

It was requested that an investigation-into the casual factors of the errors be conducted and a written response provided.

It was further stated that the out-of-service errors were similar to the errors that took place during the January 24, 1991 loss of vessel inventory event at Quad Cities Station (see Inspection Report 50-254/91006), and as such, the response could be included with the response to the issues described in Inspection Report 50-254/91006.

The telephone conversation W1th Bruce Surgess clarified that the response concerning the preparation of the out-of-service could be addressed with the out-of-service issues identified in Inspection Report-50-254/91006.

Commonwealth Edison has reviewed this event separately and in conjunction with the January 24, 1991 event to determine the root cause and identify effective corrective actions.

Regarding out-of-service personnel errors, these events are similar and, as such, will be addressed in the response to.the out-of-service issues described in Inspection Report 50-254/91006.
