Dear Mr. Collins:

This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period August 9-13, and August 30-Sectember 3, 1932. The following response to the items contained in the Notice of Violation is hereby submitted:

Functional Test Documentation of Safety-Related Chance Technical Specification 7.4.a.3 requires that " Written procedures shall be established, implemented and maintained covering . . . test activities of safety related equipment."

Licensee Procedure Q-11, Issue 3 (December 29,1981), " Test C cr.t ro l ," requires, in Section 3.0e, that " Testing practices

. employed on safety-related items are required to assure

. that . . . test results are documented and evaluated to assure tes'. requirements have been satisfied." Licensee Procedure 0-17, Issue 4 (January 13,1982), "Ouality Records," requires in Sections 4.2.3D and c, that " Records providing objective evidence of conformance are required to include, as appropriate , . . results of the inspection or test . . . dated signatures (s) of the person (s) verifying conformance or acceptability of evaluated test results."

Centrary to the above, there das n'o documentation of functional testing of the safety-related design change performed in CN 653.

l (1) The corrective steps which have been taken and the results

  • achieved: .

The licensees processing of CN's and CWP's was changed in August ISSO to give uniform treatment to FT documentation.

CN 653 did not receive the benefit of this aforementioned processing.

(2) Corrective steps which will be taken to avoid further violations:

Proper implementation of er.isting procedures for processing of CN's and CVP's.

(3) The date when full compliance will be achieved:

CN 653 was completed October 8,1981 and functionally tested via SR 5.4.9-A2 (copy attached), on October 28, 1980. Compliance was ac_ hieved as a result of the SR on October 28, 1980.

1. Connect a DTM to P-3 -(+) and TP-0 (-) on 'RIS-9301. Measure end record the discri=inator level "AS FOUND" on Data Sheet 1.

l NOTE: On all 3 vide ocdules TP-0 is below TP-1 and is not marked.

2. Connect the DVM to ~P-5 (+) and TP-0 (-) . Measure and record the high vol: age on Data Sheet 1. (This is done by using a y a= meter and measuri=g the curren: be:veen TP-5 and TP-0, 50 v amp = 1000 volts.__
3. On I-14, say 403, Flo, 115-9301 go to TRIP ADJ. Verify the "no fail" light goes out.
4. Verify the alar: I-01 C5-3 comes up.
5. Pull RIS-9301, in I-14, check the mechanical zero (the module .

indicator should read 30 cpm when the module is pulled) and bring the module to the shop.

6. Re=ove the lead shie.ld from the end and top of RT-9301 located in the Analytical Instr =nent ' Roo=.
7. Disconnect both r. ables from the detector tube.

E. Loosen bolts on pig around detector a'nd remove detector tube fro =

pig and bring detector to the shop. CACIOK: Be careful not to da= age fo'l i end of tube.

9. Setup the module and detector on the MTR-2 per Appendix 1.

10. Connect a DVM to RIS-9301. TP-1 (+) and TP-0(-). _

.11 Put node svitch to the TRIP ADJ position and adjust the TRIP RET to read 10.0 volts on the voltmeter. Check that the module reads 2 x 10 7 cp=, if not adjust METER ADJ until it does. .

  • 12. Decrease ~~ RIP REF voltage to belov 1 volt (reset trips) and then increase the
  • RIP RET voltage. Record the vo7tage levels at which the red light is 11: "AS FOUND" in the table below.

~14. Check rhe RIS =eter indicator linearity by putting the module mode switch to the TRIP ADJ position and using the TRIP REF. Record the voltage necessary to obtain the appropriate ep: readings.

7. Remove the lead shield fro the end and top of R*-9302 located on the turbine deck next to the Lunch: 2em wall at the top of the stairs.
8. Disconnect both cables from the det tetor tube.
9. Loosen bolts on pig around detector and remove detector tube from pig and bri=g detectoi to rhe shop. CAUTION: Be careful no: to da= age foil and of tuba.
10. Se:up the module and detector on the ICR-2 per Appendix 1.
11. Connec: a DVM to RIS-9302 ~P-1 (+) and TP-0 (-).

22. NOTE: the Eins and Slope may need to be repeated and adjusted.

If they agree within 1 15:: return detector and module to normal service and go to Step 23.

23. When the detector and module are returned to service, energize the check source by putting the function switch to the check-

- source position. (Hold in this position approx.15 sec.) .

14. Put mode switch to the TRIP ADJ position and adjust the TRI? REF to rend 10.0 volts on the voltmeter. Check that the module reads 2 x 107 cpe, if not adjust METER ADJ until it does.

22. Read the RIS module indicator and verify it reads the same as the average of the three counts. If these readings do not agree, adjust the SLOPE on the module until it does. .
24. Compare the "AS FOUND" data of Steps 20 and 22 to the acceptance criteria in Appendix 1. If they agree within i 152 return detector j and module to nort.21 service and go to Step 25.
25. When the detector and module are returned to servica, energize the check source by putting the fune:1on switch to the check-source position. (Hold in this position approx. 15 sec.)

6. Pull PlS-31193 in I-14, check the mechanical =ero (the module indicator should read 30 cp= vhen the module is pulled) and bring the module to the shop.
7. Disconnect both cables from the detector tube located on cesanine level east side of condenser in the air ejector discharge line.
8. loosen bolts on pig around detector and remove detector tube fro pig and bring detecter to the shop. CAUTION: Be careful not to da= age foil end of tube.
9. Setup the module. cnd detector on the MIR-2 per Appendix 1.

10. Connect a DVM to RIS-31193 TP-1 (+) and TP-0 (-).
11. Put mode svitch to the TRIP ADJ position and adjust the TFlP RET to read 10.0 volts on the voltmeter. Check that the module reads 7

2 x 10 cp=, if not adjust METER ADJ until it does.

12. Decrease TR** REF voh age to below 1 volt (reset ::1ps) and then increase the TRIP REF voltage. Record the voltage levels at which the red light is lit "AS FOUND" in the table below.
