Dear Sirs:
By this letter we report a failure to verify proper operation of an intrusion alarm within the interval specified in our Physical Securit/ Plan.
The Physical Security Plan states that the intrusion alarm including its connection to the Department of Police and Security will be checked once each week. During the week beginning November 28, 1982, no such alarm check was per-fo rmed. When a review of records on December 8,1982, revealed that no record of an alarm check had been made, the alarm was immediately checked and found to operate normally. The failure to make the required check was an oversight on the part of the individual securing the laboratory on Friday evening. Semi-annual preventive maintenance and checks of the system were performed during the week, however, since the battery discharge test and local operation was tested. These checks did not include a check that the alarm to the Depart-ment of Police and Security was verified.
The alarm to Police and Security was checked on November 26, 1982, and December 8,1982, and worked properly. A failure of the wiring to Police and Security results in an alarm at that location, so it is unlikely that the alarm system was inoperative since local operation was checked during the in te rval in questlon.
