Safety Enhancement Program - Item 55 on Very High Radiation Area Doors (VHFA)

In Reference 2, Omaha Public Power District (0 PPD) transmitted its response to NRC Inspection Report 88 05, concerning OPPD's radiation protection program.

In this response OPPD stated that:

"An electrically supervised monitored strike system will be installed on RCA doors outside of containment to alarm at a remote location if either

" door open" or " latch bolt not engaged" condition occurs... and the alarm termination points will be located in the Security Alarm Stations to provide alarm functions."

This commitment was later incorporated into item 55 of the Safety Enhancement Program (SEP), which was transmitted in Reference 3.

During the validation conducted on SEP Item 55 it was discovered that one of the affected Very High Radiation Area (VHRA) doors did not have a remote alarm for the " latch bolt not engaged" condition.

Investigation of this issue l

revealed that Modification MR FC-85 049 was responsible for installation of the hardware related to this commitment. As originally designed, both " door open",

and " latch bolt not engaged" conditions would be alarmed at the Secondary Alarm l

Station (SAS).

' Page 2 During installation and testing, problems related to electrical supervision of the alarm points were discovered, and subsequently the design was changed to give a local alarm at the respective door for the " latch bolt not engaged" condition, and a remote alarm at SAS for the "docr open" condition.

The result is that affected VHRA doors do not have a remote " latch bolt not engaged" alarm at SAS as committed to in References 2 and 3.

The commitments made relative to VHRA doors were intended to provide appropriate alarms to radiation protection (RP) personnel in the event a door is left unlocked.

Under the original commitment, security personnel would have been required to notify RP personnel of a door open condition because no local alarm was required.

However, as currently installed, RP personnel are appropriately notified of an unlocked door, or " latch bolt not engaged" condition, by the local 100 decibel horn and alarm light.

This provides adequate notification to personnel.

As a result of the above OPPD is revising its commitments related to the response to Inspection Report 88-05 (Reference 2), and SEP 55 (Reference 3) to read:

"An electrical strike system will be installed on RCA doors outside of containment to locally alarm a " latch bolt not engaged" condition.

An electrically monitored, supervised system will also be installed on these doors to alarm at a remote location if a " door open" condition exists.

Since most of these doors are fire barrier doors, special fire rated assemblies must be obtained.

These alarm systems will be installed in conjunction with MR FC-85-049, " Site Security Upgrade" and the required remote alarms will alarm at the Security Alarm Stations."

0 PPD has verified that each of the required RCA doors outside of containment have the necessary hardware to support the revised SEP 55 commitment and has functionally tested both the local and remote alarms for affected VHRA doors.

The revision to the controlled SEP manual to reflect the change to SEP 55 will be issued within 30 days of the date of this letter.
