Section 10.5 of Callaway's Supplemental-Quality Assurance _ Program

'for Fire 1 Protection requires that emergency lights be periodi-cally-inspected'and tested to ensure acceptable conditions.

-Contrary,to the above, as of-November 9, 1990,. emergency lights were_not beingimaintained or tested to ensure they;would function as designed. This resu?.ted in the failure of one light and ques-tionable operability of eleven others due to low electrolyte

- i Currectivo actions that have been taken and the results achieved:

Work requests have been completed to correct the twelve fire protection emergency light problers. The failed light had low electrolyte and a blown DC output fuse, ten others had low elec-trolyte, the twelfth light had corrosion around the vent cap. The fuse was replaced on the failed light. Fluid was added as required to eleven lights. The corrosion around the vent cap was removed. Union Electric considers the emergency lights to be operational.

The correctivo actions that will be taken to avoid further violations:

i The current annual PM will remain unchanged. An additional PM for 1

emergency lighting will be written to check electrolyte level and verify light operation. The additional PM will be performed three times a year. Thus each light will be verified operational and the electrolyte level will be checked on a quarterly basis.

During PMs-the as-found condition of the lights will be recorded.

~ The frequency of PMs will continue to be evaluated and adjusted as required to maintain light operability.

The date when full compliance will be achioved:

Performance of the annual PMs should be completed by May 1, 1991.

The additional PM will be written and ready for implementation by July 1, 1991.
