Torquing Square D Breaker Lugs
Dear Mr. O'Reilly:
On August 20, 1982, Mr. Virgil Brownlee of the NRC Region II was. Otified of a reportable significant deficiency concerning safety related Square D nolded case breaker lugs in 480V motor control centers and panels not torqued to values specified. During a recent inspecticn of a Square D Conpany breaker installation, the CC inspector raised the question of torquing the breaker lug to 35 in-lbs. This breaker had a manufacturer's sticker that stated that for copper and aluminum cables, the lug should be torqued to 35 in-lbs.
NCN 817 was generated by the QC department on the torquing of Square D nolded case breakers in notor control centers and panels.
The Square D manual for the notor control center states that nolded case breaker lugs should be torqued to specific torque settings as stated in the manual table.
A sanple inspection was perforned, and an excessive number of lugs mre found that failed to meet specifications. Therefore, 100 percent inspection and retorquing ms performed.
South Carolina Electric and Gas Ccmpany has determined that this item is reportable under 10CFR50.55e. This letter and the attachcd significant deficiency form serves as a final report of this natter.

Number and Location of Nonconformance Breakers in DCIDA2X, XIClDA2Y, DClDA2Z, DClDB2X, DClm2Y, DClDB2Z, XMClEAlX, XMClmlX, DClEClX, and APN panels.

Significant Deficiency Created and Evaluation
'Ibe breakers operated properly electrically, homver, the barrel ltys were not torqual to required settirgs. There was a probability that ene or acre of the breaker connections could have cuerheated, creating a potential for a fire.
4.
Corrective Action Teminations to " Square D" trolded case breakers req; ire torquing to the manufacturers recomended value.
(Each breaker is taggal with the reccmmoded minimum IB-IN. valtn). All future terminations to these breakers should be made per Em 300.2 steo 7.1.5 using the torque value stated on the breaker.
