Prompt Notification of a Reportable Occurrence File: A-118a, TE-4
Dear Mr. Keppler:
This letter, telecopied to your office, is intended to satisfy the requireinent for prompt notification of a Reportable Occurrence in accordance with Specification 6.11.2.a of the Duane Arnold Energy Center Technical Specifications.
Technical Specification paragraph (s) violated:
3.5.0.
Description of Occurrence:
During normal operation on 9/12/82, the HPCI inboard steam supply isolation valve M0V-2238 closed due to a false leakage signal from malfunctioning temperature differential switch TDS-2260. A 7-day LC0 was entered per Technical Specification (T.S.) 3.5.0.2.
The isolation signal was cleared and HPCI operational testing was begun. During this testing, HPCI fast-started in approximately 38 seconds instead of the required 25 seconds.
HPCI was declared inoperable and the operability of the other ECCS subsystems

was tested. During testing, the RCIC turbine tripped at rated speed.
A 24-hour LCO was declared per T.S. 3.5.D.3 and reactor shutdown commenced. The 24-hour LC0 was reduced to a 7-day LC0 after approximately 6-3/4 hours when RCIC was tested satisf actorily and declared operable. HPCI was operability tested satisf actorily but the 7-day LC0 was not lifted per management decision. At 1020 on 9/13/82, the RCIC turbine tripped during daily surveillance. A 24-hour LC0 was entered per T.S. 3.5.D.3 and reactor shutdown SO g T3B?.

The 24-hour LC0 reverted to a 7-day LCO after approximately 4-1/2 hours when HPCI was tested satisf actorily and declared operable. The 7-day LCO was ended 9/13/82 at approximately 1600 when RCIC was declared operable.
