Root Cause Evaluation Plan Page1 1.0 Introduction At the request of the Director Millstone Unit 2 (MP2), a formal root cause investigation of the operational events which resulted in three late Unusual Event Notifications between April 20-23,1994 at MP2 will be performed in accordance with NEO 3.15, " Root Cause Evaluation".

2.0 Problem Statements The Root Cause Evaluation Team will evaluate the following:

i 2.1 MP2's ability to assess, classify and analyze events, and make the appropriate notifications.

2.2 MP2's ability (i.e. management, operations, etc.) to recognize the significance of events.

3.0 Sropte _

3.1 Review the operational aspects of the April 1994 events. Specifically:

a. RCS Leak Event (PIR 2-94-143)
b. Stuck Control Rod Event (PIRs 2-94-144, -145, -146)
c. Insufficient Shutdown Margin Event (PIR 2-94-147) 3.2 Review the Emergency Preparedness aspects of the April 1994 events (i.e. failure to meet MP2 Technical Specifications and the failure to properly use MP2's Emergency Action Level (EAL) Tables) which resulted in three late Unusual Event Notifications between April 20 - 23, 1994.

3.3 Assess the adequacy of MP2's root cause capability.

3.4 Assess the ability of MP2 to identify and ameliorate Control Rod Drive System malfunctions.

scope need to be expanded to ensure for proper review, the Director - MP2 and any affected department managers will be promptly notified in accordance with NEO 3.15 Section 6.4.
