Bulletin No. 82-04

" Deficiencies in Primary Containment Electrical Penetration Assemblies" Shoreham Nuclear Power Station - Unit 1 Docket No. 50-322

Dear Sir:

The subject NRC Bulletin describes potentially signifi-cant problems pertaining to electrical penetration assemblies (EPAs) supplied to several nuclear power plants by Bunker Ramo Corporation of Chatsworth, California.

These EPAs have been reported to have problems involving wire terminations performed by the vendor.

These problems included improper lug crimps, incorrect lug types, loose connections on terminal blocks, impro-perly identified sanetration cables and cracked insulation on conductors as they enter / exit the epoxy modules.

A review of the EPAs at Shoreham has revealed that Bunker Ramo Corporation EPAs are not utilized here.

General Electric Series 100, medium voltage and Series 200, low voltage EPAs are utilized at Shoreham.

In addition, some low voltage EPAs supplied by the Conax Corporation are installed in General Electric Series 200 penetration headerplates.
