Interim Report of 10CFR50.55(e) Item on Loose Holding Stem Nut Keys on Remote Operated Powell Valves (#72)

Dear Mr. Keppler:

This interim report on the loose holding stem nut keys on remote cperated Powell valves problem has been prepared to provide the current status of this problem.

This item was originally reported to Mr. P. Pelke of NRC Region III by Detroit Edison's Mr. H.A. Walker, Supervisor-Construction Quality Assur-ance, on June 21, 1982.

Two failures of Powell motor operated valves were experienced at the Fermi 2 site. The failures were caused by the stem nut holding keys falling out of place causing the valve not to operate. These Powell valves are nodel 190Sl-Y-NE, with Limitorque Operator SMB-5T. The two valves are Fermi 2 valve nturbers V8-2159 and V8-2160, and are in the Residual Heat Removal (Ell-00) System.

A determination has been made by Powl1 Valve Co. that the use of retaining plates, and the usual staking of the keys, will alleviate the problen's en-countered with valves V8-2159 and V8-2160.
