POTENTIAL 10CFR50.55(eJ DEFICIENCY RT INDICATIONS, SAFETY INJECTION TANKS AND REACTOR PUMP SUPPORTS (D/N #23)

Reference:

Letter, G03-82-831, dated August 20, 1982, Mr. R. S. Leddick to Mr. D. M. Sternberg, same subject.

Ebasco, in conjunction with Combustion Engineering (CE), was directed by the Supply System to assure an evaluation of the safe-end indica-tions (associated with the subject deficiency) was performed per 10CFR50.55(e).

The referenced letter reported that this evaluation was not complete; however, upon completion, the results would be forwarded to your office by October 22, 1982. To date, the evaluation has not been completed.

It is anticipated that the results will be provided to your office by January 14, 1983.

The status of repair actions taken is as follows:

o CE has submitted an ASME Section XI repair plan documenting the rework required per the disposition of the original NCR.

e The repair plan was approved by the Supply System and the work commenced on September 13, 1982.

e Grinding operations revealed additional indications that were not indicated on the RT film. CE elected to remove the indica-tions rather than evaluate the relevancy of the indications.

e Final PT of the excavation areas was accepted by CE QA Representa-tives and the Supply System on October 13, 1982.

A-Mr. D. M. Sternberg October 26, 1982 Page 2 G03-82-1096 e Excavated areas were fully mapped on the table attached to the repair plan to show the length, width and depth of the indications.

e Carbon steel base metal was encountered during the grinding operations.

Therefore, an additional CE welding procedure was required. The procedure was approved by the Supply System and added to the repair plan.

e The welding, required to build up the excavated areas, commenced on October 13, 1982.

It is expected that the weld repair and PT will be completed by the first week of November,1982.

e CE's initial evaluation has determined that the defects were not significant (i.e.,.not reportable per 10CFR50.55(e) ).
