NRC COMBINED INSPECTION 50-272/82-19, 50-311/82-19 SALEM GENERATING STATION UNITS NO. 1 AND 2 AUGUST 4 THRU AUGUST 31, 1982 The following is the station response to the notice of violation:

ITEM OF VIOLATION Item A.1 Technical Specification 6.11 and Administrative Procedure 24, Radiological Protection Program, part 6.9.3 Personnel Surveys, require that personnel shall monitor themselves when leaving the station.

Contrary to the above:

1 On August 8, 1982, personnel were observed leaving the station through the contractor gate house with no operable monitoring equipment at such egress point.

l Reply to Item A.1 On August 8, 1982 a power failure was experienced at the Salem Auxiliary Guardhouse, causing a failure of the installed portal contamination monitors.

Security notified the Radiation Pro-tection Department, and three Eberline RM-14 portable monitors 8211020055 821028 gDRADOCM 05000272 PDR I

Nuclear Regulatory Commission 10/8/62 were delivered.

Prior to delivery they were source and battery checked, and verified in good operational condition.

However, because no A.C.

power was available, they were operated on their batteries, which eventually were drained, causing the failure of the instrument.

During the period that the monitors were in-operable, personnel exited the station through the Auxilairy Guardhouse unmonitored.

When the battery failures were dis-covered, the monitors were replaced with operable ones by Radiation Protection.

A training course for senior security personnel will be developed to qualify them on proper usage of portable monitoring equipment, and basic operational troubleshooting.

Secondly, the instrument issue procedure (RP 1.004) will be revised to include the follow-ing caution:

CAUTION:

RM14's should not be issued to non-qualified personnel when A.C.

power is not available.

The E-140 with frisker probe may be used as an alternate.

This action will be implemented by October 31, 1982.
