IE Bulletin 80-06 Reference (A). Boston Edison Letter dated June 27, 1980 from Mr. G. Carl Andognini to Mr. Boyce H. Grier (B): LER 82-025/01T-0
Dear Sir:
i In.the second response contained in Reference (A), Bo-ton Edison committed to complete IE Bulletin 80-06 related surveillance testing prior to start-up from Refueling Outage #5 (RF0 5). At start-up from RF0 5 we believed this commit-ment had been fully satisfied.
However, after Pilgrim returned to service an additional review was conducted i
to ensure the adecuacy and completeness of the Bulletin 80-06 tests. This re-view determined that a modified station surveillance procedure, Temporary l
Proced_ure 81-64, PCIS Valve Testing (TP81-64) had the following inadequacies:
(1) it inadvertantly overlooked the testing of the H2 Analyzer Isolation Valves; l
(2) it did not identify the individual Standby Gas Treatment System Isolation
(
Valves; (3) it incorrectly indicated that the 02 Analyzer Valves are closed after i
reset, when the valves are actually open.
To correct the identified inadequacies, the Operations Review Comittee approved two new temporary procedures, TP82-38, Balance of Plant isolation "A" and TP82-39,
(
Balance of Plant Isolation "B". During the unscheduled shutdown commencing August 13, 1982, th ! additional testing was conducted.
This cesting revealed that wiring l
to the H2 Analyzer valves was incorrect. We developed and' implemented corrective i
actions, developed appropriate new test procedures and conducted a test of the logic.
L The logic tested consistent with its wiring schemes, satisfactorily concluding this

t test.
LER 82-025/01T-0 has been issued to satisfy reporting requirements.
