.J SPECIAL REPORT SR-94-002-00 Steam Generator Eddy Current Examination (6th Refueling)-

On March 28, 1994, Waterford 3 completed 100% inservice eddy current bobbin coil full length examination of the Steam Generator (S/G) tubing..This.

report is submitted in accordance with Technical Specification (TS) 4.4.4.5.a, which requires reporting the number of tubes plugged in each S/G within 15 days of completing Inservice Inspection (ISI) of S/G tubes pursuant to TS 6.9.2.

A follow-up report will be submitted in accordance with TS 4.4.4.5.b by March 28, 1995.

The total number of tubes which have been plugged in each S/G are shown below:

  • Row 121, Line 45 leaking Westinghouse I-690 mechanical plug - reworked under NCI-290326.

Babcock and Wilcox Nuclear Services (BWNS) machined S/G

  1. 1 hot leg tube plug location Row 121, Line 45 to accept a BWNS 0.750 tapered weld plug. Work was completed and inspected on March 28, 1994.

H CR-94-308 was written on March 31, 1994 to evaluate root cause of plug failure. The details concerning this plug failure will be included in the follow-up report to be submitted in accordance with TS 4.4.4.5.b by March 28, 1995.
