Interim Report of 10CFR50.55(e) Item on 1" 3000# Carbon Steel Pipe Couplings and 1" 3000# Pipe Caps (#77)

Dear Mr. Keppler:

On August 10, 1982, Project Quality Assurance's Mr. E.L. Thcmpson, Acting Supervisor-Construction Quality Assurance, telephoned Mr. J. Konklin of NRC Region III, to report on a problem with 1 inch diameter, 3000 PSI 7

pipe couplings at the Fermi 2 site.

This item deals with twelve (12) rejectable couplings fran a sample of twenty (20) located in the warehouse. The couplings were manufactured by Capital Manufacturing Ccmpany and carry Heat Code Number 0680. Linear indications were observed visually, and confirmed by liquid penetrant examination (PT) . Seventy-seven (77) couplings have been issued for in-sta11ation. Investigation is being conducted to locate where these sus-pected couplings were installed so that visual inspection and PT (if nec-essary) can be perforned. Unused couplings are to be returned to the manufacturer.

Also, during preliminary investigation, twenty-five (25) caps were examined and wre found rejectable by liquid penetrant testing. These caps were also manufactured by Capital Manufacturing Ccupany and carry the same heat number as the couplings. Of the 84 remaining caps, it is probable that an indeter-minable amount was installed in non-ASME, non OA I systems, which would prevent accounting for all 84 caps. The investigation is continuing.

Contractor docunent review verifies the heat number of each OA I and/ar ASME piece of material via the heat number card file. The card for this heat number (0680) has been annotated "RFJECT" to identify to the document reviewers in the future that this material is nonconforming. to prevent release for construction.
