liNCLOSURl! TO NYN-94030 INOPERABI E SEISMIC MONITORING INSTRilMENTATION f

On March 4,1994 seismic response spectrum recorder 1-SM-X-6706 was removed from service to perform the 18 month Channel Calibration pursuant to Surveillance Requirement 4.3.3.3.1.

Recorder I-SM-X-6706 is located in the Containment at the -26 foot elevation. Upon opening the recorder to commence the calibration corrosion and moisture damage was noted. Specifically, numerous sensors were corroded and their ability to accurately record seismic activity was called into question. The damaged sensors were replaced and recorder 1-SM-X-6706 was restored to operable status on March 17,1994.

On March -16, 1994 it was determined that recorder 1-SM-X-6706 had been inoperable for an indetenninate period of time. Therefore, this occurrence is being reported pursuant to. Technical

)

Specifications 3.3.3.3, Seismic instrumentation, and 6.8.2, Special Reports.

'I T he seismic response spectrum recorders are sealed with rubber gaskets on the cover plates and all bolts-

{

penetrating the recorder case are sealed with silicon. The apparent cause of the corrosion is exposure to j

water or moistnre from an indeterminate sonrce. Two other seismic response spectrum recorders located j

in the same area in the containment did not exhibit similar corrosion. The cause of the excessive corrosion in recorder 1-SM-X 6706 will be evaluated in accordance with the North Atlantic corrective i

action program.
