Dear Mr. Seyfrit:
On August 17, 1978, we reported to Mr. Tom Westerman, Region IV, the failure of a weld in a drain line in "A" LPSI loop as a potential signi-ficant deficiency. The section of pipe containing the failed weld has been removed and replaced. The final report is attached.
