Waterford SES Unit No. 3 Supplemental Report to USNRC Inspection Report 50-382/Rpt. 82-10

Dear Mr. Madsen:

In LP&L letter W3I82-0040, LP&L committed to submitting a supplemental response by November 15, 1982 to the USNRC Inspection Report 82-10.

The following information is submitted as the supplemental information to Inspection Report 82-10, Infraction No. 1.

1. Failure to Follow Instructions for Installation of Safety-Related Instrumentation Systems Criterion V of 10CFR50, Appendix B, requires that activities affecting quality be accomplished in accordance with instructions, procedures, or drawings.

Ebasco Construction Specification IC-1, Revision 1, " Instruction for Erection of Instrumentation Systems", paragraph 5.3, requires that instru-ment sensing lines slope continuously toward the instrument with k inch per foot minimum.

2.

Drawings LOU 1546-B-430 (dated March 19, 1979), " Instrument Installation Details" General Note 4 on Sheet 1 of General Notes requires that instrument sensing line slopes be \ inch per foot minimum.

Contrary to the above, the impulse tubing lines for the steam generator level instrument variable legs as shown on the installation isometric drawings and as installed do not meet the minimum sloping requirement.

The sections of instrument tubing which do not meet this requirement are from each root valve (8) at the steam generators to the point where the instrument tubing penetrates through the concrete shield wall.

This is a violation.

Madsen W3182-0018 Page 2 Corrective Action Taken and Results Achieved Field Change Request No. IC-P-469 was issued to modify the installed supports for Steam Generator Level tubing and tube track to achieve the proper installation to meet design criteria. Additionally, Design Change Notification No.NYC-IC-1114 was issued to install the Steam Generator instrument tubing to provide a continuous slope of not less than \" per foot thus satisfying design criteria.

Actions Taken To Preclude Recurrence System Walkdowns are being conducted to identify installations which have been damaged or deviate from design criteria. Each deviation is evaluated to determine the acceptability or need to rework.

Date When Full Corrective Action Will Be Achieved Physical work on the Steam Generator level instrument installations will be completed on or before December 31, 1982.
