During several recent inspections of the drywell steel, you have called to our attention your concerns over three separate conditions.

'Ihese conditions have been investigated and this letter is our response to those concerns:

1.

Tbrquing of bolts at radial beam sliding connections:

At elevation 590 of the drywell, two radial beams were found with deficiencies at the end connections. We have initiated a non-conformance report (copy attached) to cover the discrepancies. The disposition of the non-confonnance calls for upsetting the threads of the bolts or jam nuts to prevent the ranoval of these nuts in the future. We will inform you after this non-conformance is processed and issued to the field for work.

At elevation 561 of the drywell, the trp and bottcm flanges of beam 104 have been co[xxl to provide cicarance for an MSIN line. 4hese beam copes are called for on detail 22 of S&L drawing S-399, Pcv. BB.

Ilowever, the detail permits a maximum cope of one-inch. The actual copes measure 1-1/8 in and 1-3/8 in. We have initiated another non-conformance (copy attached) to cover this condition.

At elevation 535 drywell,14 in. pipe sleeves have been installed through the webs of beams 129 and 138. These sleeven were installed to accamrxlate the routing of instrumentation lines. The concern is whether or not these sleeves were covered by design drawings.

%e sleeve in beam 129 is shown as hanger number 1NX2528HG for line 1NBC1AD3/4 (see DDC M-3525 attached). h is hanger is also shown on S&L drawings M-471, sheet 60 and M-471, sheet 40. W e sleeves in beam 138 are covered under DDC's M-11,238 and M-11,300 as hanger numbers 1NX2510HG, INX2520HG and 1NX3937HG. Hanger numbers 2510 and 2520 have been picked up on S&L drawing M-471, sheet 60. Copies of the above DDC's are attached.
