Waterford SES Unit 3 Docket No. 50-382 Construction Permit No. CPPR-103 IE Inspection Report No. 50-382/78-12

Dear Mr. Seidle:

LP6L's response to the item of noncompliance identified in the subject report is given below:

Failure to Properly Inspect Structural Welds 10 CFR 50, Appendix B, Criterion X requires that a program for inspection of activities affecting quality be executed to verify conformance with procedures.

The American Eridge Procedure No. 6, Revision E, paragraph 6.8.2 requires that weld profiles be as shown in Figure 3.6 of the American Welding Society (AWS) Structural Welding Code Dl.1 and that undercut be no greater than 0.03 inches.

Contrary to the above:

One full penetration weld and two fillet welds, at elevation (-)4' in the Containment Building structural steel, has been visually inspected and accepted by American Bridge, but were found to have undercut of approximately.06 inches and overlap and slag in excess of the acceptance criteria specified in American Bridge Procedure No. 6 and the AWS Structural Welding Code DI.l.

Corrective steps takea and the results achieved:

Nonconformance Report (NCR) W3-1072 was generated identifying full penetration and fillet welds at elevation (-)4 ' of the Reactor Con-tainment Building as possibly not meeting AWS Code Requirements.

A reinspection of welds identified by the disposition to NCR W3-1972 was made by American Bridge inspection personnel.

Welds not meeting AWS Code requirements as well as welds possibly not meeting Code re-quirements have been identified.

Those not meeting Code requirements will be reworked as necessary.

Corrective steps which uill be taken to avoid further noncompliance:

Accrican Bridge personnel performing any weld inspection have received additional training by the American Bridge General QA Of fic.er.

This training covered the subject of visual acceptance criteria for com-pleted welds.

American Bridge foremen and welders received additional training as to the acceptance criteria to be met on completed welds.

Full compliance will be achieved'by January 5, 1979.
