I This letter is submitted to document a discrepancy associated with NRC inspections invoice number SS0088-94.
On January 25, 1994, Waterford 3 received a facsimile of invoice SS0088-94 from our E01 corporate office.

The invoice had been addressed (by the NRC) to the attention of Mr. Jerrold j
G. Dewease instead of (as previously practiced) to the attention of Mr.
R.F. Burski.
Results of our internal review of this matter indicate that the invoice was apparently initially received on site and then routed to Mr. Dewease's corporate office.
Once it was determined that the invoice was for payment by Waterford 3, the invoice was faxed to Waterford 3 for further processing.
Upon receipt of the fax, Waterford 3 immediately wired payment of the invoice to the NRC (January 25, 1994).
Subsequently, Waterford 3 has received a notice of late payment dated January 24, 1994 140034 g

4 Part 170 Facility Inspection Invoice Number SS0088-94 W3F1-94-0110 Page 2 March 4,1994 (also addressed to Mr. Dewease's attention) and another dated February 1, 1994.
The latter notice is an NRC request for payment of accumulated interest associated with late payment of NRC inspections invoice SS0088-94, Upon review of the circumstances associated with this transaction, Waterford 3 is respectfully requesting a waiver of the accumulated interest charges.
Waterford 3 wired payment of the accumulated interest charges to the NRC on February 28, 1994.
We have no record of an E0I or Waterford 3 request to change from the previously practiced mailing of inspection invoices to the attention of Mr. R.F. Burski.
It is our understanding, through discussions with your personnel that the NRC also has no record of an authorization to make the change.
Your office does have a record of a call from Waterford 3 personnel (January 25,1994) asking to have future inspection billings made to the attention of Mr. Ross P. Barkhurst.
However, this contact (by Waterford 3 personnel) was made after it was determined that the previously practiced mailing arrangement (to Mr.
Burski's attention) had somehow changed.
Since your system allows changes of billing addressee by telephone, any earlier established arrangements become a mute point.
Waterford 3 does have a record of contacting the NRC in a letter dated July 19, 1991 (W3F1-91-0379) requesting that Mr.
Dewease's name be removed from the Waterford 3 NRC Service List due to changes in his responsibilities.
We ask your consideration in this matter.
In addition, we are hereby requesting in writing that future Waterford 3 invoices (inspections, amendments, Annual Fees, etc.) be sent to Waterford 3 to the attention of Mr. Ross P. Barkhurst, P.O. Box B, Killona,- La. 70066 unless a change request is officially submitted in writing from Waterford 3.
