Dear Mr. Crocker:

In response to your letter of September 26, 1978 and in accordance with 10 CFR 2.201, the attached reply addresses the Netice of Violation which was included as Appendix A of the referenced Inspection Report. The noted violation was the failure to fully post and barricade a high radiation area.

Although this noncompliance appears to be a recurrent item, when examined in detail it is apparent the specific noncompliance was unique in both the problem and its necessary resolution as discussed in the attached reply. We do, however, f-share your concern about the occurrences of noncompliances in our radiation control program. Please be assured that this matter and its resolution is receiving our utmost attention.

REPLY TO NOTICE OF VIOLATION

Description Of Infraction (78-22-01)

Technical Specification 6.12. "High Radiation Area", states in Section 1.A.

"In lieu of the ' control device' or ' alarm signal' required by paragraph-20.203(c)(2) of 10 CFR 20... Each.High Radiation Area in which the intensity j

ff of radiation is greater than 100 mrem /hr but less than 1000 mrem /hr shall be -

barricaded and conspicuously posted as a High Radiation Area and entrance thereto shall be controlled by issuance of a Radiation Work Permit and any individual or i

group of individuals permitted to enter such areas shall be provided with a radiation monitoring device which continuously indicates the radiation dose rate in the area."

Contrary to this requirement, on August 31, 1978, a portion of elevation 718 in the Reactor Containment in the vicinity of the Pressurizer Relief Valves had radiation intensities as high as 140 mrem per hour in the general area, but was i

not barricaded or posted as a high radiation area. Although some posting and l

barricading had been done, it was possible to enter the area without passing

through a barricade or observing any posted signs.

Discussion Of Infraction l

When the unscheduled station shutdown occurred on July 28, 1978, all areas of the Reactor Containment were surveyed to establish radiological control requirements for entry and work performance. At this time, all Reactor Containment areas were posted and barricaded as required by Technical Specification 6.12.

On August 31, 1978, it was learned that a temporary barrier which was installed at elevation 718 of the Pressurizer Compartment had been moved and not properly

re-established. This then-created another access (via circular stairway from other levels) to the subject high radiation area.

Corrective Action l

The temporary barrier and posting signs were properly re-positioned to preclude entry to the high radiation area via the circular stairway.

Reply To Notice Of Violation Inspection No. 78-22 Letter Dated September 26, 1978 Page Action Taken To Prevent Recurrence 1.

The circular stairway will be enclosed with expanded metal screen and doors at the 718 and 738 levels of the Pressurizer Cubicle.

This work is expected to be completed by the end of the current shutdown.

2.

Exclusion doors will be maintained locked and posted as required by-Technical Specification 6.12.

If plant evolutions in the area require doors to remain open, a more secure temporary barricade (i.e. locked chain) will be installed.

3.

During Station outages, there will be an increase in Radeon surveillance of the posted high radiation areas to verify that the barriers are correctly positioned.
