JOSEPH M. FARLEY NUCLEAR PLANT UNIT 1 NARRATIVE

SUMMARY

OF OPERATIONS OCTOBER, 1978 Following completion of a scheduled unit outage for maintenance and plant modifications, the unit was brought on line October 5.

The unit was operated at reduced power (350 MW - 370 MW) until October 8, due to damage to the condensate 1B, 1C motors because of possible insufficient cooling during startup.

On October.25 a unit shutdown resulted from an automatically initiated reactor trip due to an apparent voltage spike on the 1A inverter (120 V vital AC bus lA).

The remainder of the month the unit was operated at 100%

power.

The following significant safety related maintenance was performed in October:

1.

Completed replacement of 1B accumulator vent / relief piping.

2.

Investigated and corrected problem on solid state protection system train B 48V D.C. power supply 1.

3.

Performed preventive maintenance on 1C diesel generator.

4.

Rebuilt air compressors on 1B and 1-2A diesel generators; rebuilt Service Water B compressor.

5.

Removed No.10 river water pump for inspection / repair following indications of high vibration.

6.

Replaced seal on lA boron injection system recirc pump; replaced mechanical seal on 1C diesel generator jacket water pump.

7.

Replaced light diffusion disc on the A train chlorine detector.

8.

Investigated and corrected problem with failure of 1-2A diesel generator to tie on-line within required time limit.

9.

Investigated and corrected problem with failure of 1C diesel generator output breaker to close on bus 1H while performing SI/LOSP Surveillance Test.

10.

Completed repair / retest on B train penetration room filtration system after a review indicated i

initial surveillance test data was unsatisfactory.

11.

Added oil to reservoir for hydraulic snubber MS-R227 after inspection indicated oil level was low.

12.

Corrected high voltage drift problem on source range channel N-31; corrected bistable problem wih source range channel N-32.

13.

Replaced and recalibrated NLP card for steam line delta-P channel PT484 after it failed low.

14.

Disassembled 1C charging pump to correct alignment problem.
