-o Significant Deficiency Report LPSI Pump Flotor Failure (Rev. 1)

(File: 2-0520)

Gentlemen:

We stated in the subject report, attached to our September 21, 1978, letter, that we had inspected LPS1 pump motor 2Ph!60A for proper bhar --

ing ins.allation. This was an error that was made because of an Tn-1; ternal n.iscommunication.

Pump motor 2PM60A was not inspected for!.I g

proper bearing installation.

l- :

q The decision not to inspect this motor was-based on the followi'ng 7

reasons:

.I

- 2PM60A had been in continuous operation following

=

the failure of 2PM60B.

~

- 2PM60A had been operating in all possibic modes such that if a bearing problem existed the pump would have aircady been destroyed.

We feel that the reasons listed above were sufficient for not inspecting the pump bearing.

A corrected report is attached.

Very truly yours,

~
