This report has been prepared using the January,1992 NUM ARC form and accompanying instructions.
Alsa attached is a corrected report for the period of January 1,1993 - June 30,1993. This information was previously transmitted verbally n Mr. Loren Bush of the NRC by Mr. Juan Vigil of our staff.
