Violation:

I As a result of the inspection conducted at the Seabrook Generating Station on August 20 i

24, 1990, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was ident!fied:

1 Technical Specification 6.11.2 requires that areas accessible to personnel with radiation levels greater than 1000 mR/br at 18 laches be provided with locked doors to prevent unauthorized entry into the area.

t Contrary to the above, the north door to the Demineralizer Alley, located on the 7' elevation l

of the Primary Auxiliary Building, was not locked during the period July 31,1990, (9:48 a.m.)

(

through August 1, 1990, (7:30 a.m.) and radiation levels, accessible to personnel, up to 1,500 mR/hr at 18 inches were present in the Demineralizer Alley, i

Resoonse:

New Hampshire Yankee concurs with the violation as written. The event related to this i

violation was discovered by New Hampshire Yankee and was reported to the NRC in LER 90 020 00.

Additionally, New Hampshire Yankee discovered a similar event on April 19, 1990, and reported it to the NRC in LER 90 013 00.

The activities associated with the events have been evaluated to determine the cause of the violation. The cause was attributed to personnel error involving a lack of attention to detail.

l The following corrective actions have been taken.

1.

Functional tests of the intrusion alarms, door latches and closure mechanisms were conducted on all locked, high radiation area doors outside of containment to ensure operability.

2.

The personnel involved with the violation have been counselled on the events and the need for increased attention to detail.

3.

Meetings were held on August 3 and 23,1990, with Health Physics Operations Technicians to discuss the events and their potential consequences, and to. discuss I

the need to physically check high radiation area doors to ensure they are properly secured.

l 4.

New signs providing additional specific instructions for proper ingress and egress I

have been posted on high radiation area doors and barriers.

A memorandum from the Executive Director Nuclear Production has been issued e

to all site personnel referencing the events and re emphasizing the need to strictly adhere to radiation protection procedures. Managers and supervisors were directed to emphasize radiation protection concerns and requirements with their workers during routine department meetings and in daily work activities.

As a result of our ongoing review and an event involving another locked high radiation area door found unsecured on September 27,1990, the following additional corrective actions were i

expedited.

1.

Key card access authorization to locked high radiation areas was removed for all personnel.

2.

A procedure was developed and approved on September 28, 1990, specifying the new requirements for access to locked high radiation areas. These requirements include:

Personnel requiring access must obtain a controlled key from the health physics control point.

A second individual will accompany the ladividual requiring access and will be responsible for performing an independent verification of initial door closure, i

The second individual will then await the nturn of the individual which accessed the area. Upon final door closure and icdependent verification that the high radiation area door is indeed locked, both personnel will be required to sign a form indicating the door has been properl1 secured.

3.

A design change to install a padlock mechanism on current locked high radiation area doors, except for the containment peraonnel batch controls and areas inside e

containment, has been completed. The addition of this second lock is expected to provide increased awareness to personnel operating these doors.

4.

The individual involved with the September 27th event was counselled and received disciplinary action, implementation of the above corrective actions is expected to preclude problems of this l

nature from recurring.
