Dear Mr. Streeter:
PRAIRIE ISLAND NUCLEAR GENERATING PLANT DOCKET NOS. 50-282 AND 50-306 In response to your letter of September 2, 1982, which transmitted Inspection Reports 50-282/82-13 and 50-306/82-13, the following is offered:
Violation Technical Specification 6.5 states, in part:
" Detailed written procedures, including the applicable checkoff lists and instructions, covering areas listed below shall be prepared and followed
. A.4 Surveillance and testing requirements that could have an effect on nuclear safety.
Contrary to the above, on August 27, 1982, Surveillance Procedure SP-ll86 Diesel Generator Operability Test was completed and D-2 Diesel Generator was returned to service without completing paragraph 4.41 of the procedure which requires independent verification of restoration to service.
This allowed the Auto / Manual switch for the Bus 16 breaker to remain in Manual position for approximately one hour and fifteen minutes before the condition was recognized and corrected by the licensee.
This is a Severity Level IV violation (Supplement I).

Mr. J. F. Streeter September 28, 1982 Responce The switch position was corrected upon discovery by the relieving shift.
Diesel generator test procedures will be revised based on thorough reviews and operator walk-throughs on a full scale control room mockup.
This will improve the procedure emphasizing switch position and independent verification.
Procedures will be revised by December 1, 1982.
In addition, the investigative report will be discussed with control room operators emphasizing the importance of completed procedure review and independent verification.
