BEAVER VALLEY POWER STATION - UNIT 2 DUQUESNE LIGHT COMPANY FINAL REPORT ON DIESEL GENERATOR SHAFT SPIDER ASSEMBLY 1.

SUMHARY Beloit Power Systems (BPS) has reported that the Emergency Diesel Generator sets, assembled with Colt Industries (Colt) diesel engines and BPS Frame IX generators, may have loose stud and nut combinations (fastene;s) on the shaft spider assembly because of possible misapplication of manufacturers torquing equipment. A loose fastener has the potential for further loosening with an attendant risk of generator damage.

IMEDIATE ACTION TAKEN Upon notification of the problem by BPS, an inspection of both l.

BVPS-2 Emergency Diesel Generator sets was carried out to determine whether the above condition existed. BPS has informed the NRC l

(Region III) of the above problem.

3.

DESCRIPTION OF DEFICIENCY Each BPS Frame IX generator is equipped with 14 fasteners which hold the generator rotor to the generator shaft. Upon inspection at the jobsite, using BPS provided procedures, it has been determined that most fasteners were torqued at the factory to a lower than acceptable value.

4.

ANALYSIS OF SAFETY IMPLICATION The Emergency Diesel Generators are required, during design basis events, to supply electric power to the safety-related ' components.

The potential for further loosening of the fasteners under operating l

vibrations and load cycling could cause the diesel generator to i

be unavailable or fail while carrying emergency loads and, therefore, could adversely affect the safety of operations.

5.

CORRECTIVE ACTION TO REMEDY DEFICIENCIES Retorquing of all the fasteners to the BPS's specified value of 5,000 ft-lb, has been performed. Degree of rotation, of each nut, has been documented cnd will be forwarded to BPS for record and further evaluation.
