This letter serves as notification of a change in the i
physical condition of the Reactor Operator identified below.

Manley 55-61738 OP-11075 On December 15, 1993, this individual was involved in an off-duty accident which resulted in a change in the individual's physical condition such that his condition no longer meets the requirements of 10CFR55. 33 (a) (1) as determined by PECO Energy Company's Director-Occupational Healtn and Safety and Limerick Generating Station's Physician's Assistant based on the standards for general health set forth-in American Nuclear Society Standard ANSI /ANS 3.4-1983, " Medical Certification and Monitoring of Personnel Requiring Operator Licenses-for Nuclear Power Plants."
This diagnosis was-made on January 10, 1994, based on medical information provided by the individual's attending physician.
The individual is currently out of work on disability leave as a result of the accident; however, the individual was relieved of all licensed duties by plant Operations management upon notification of the diagnosis.

Us S. Nuclear Regulatory Commission February 4, 1994 Document Control Desk Page 2 Accordingly, this notification is being submitted to the NRC within 30 days of Icarning of the diagnosis in accordance with the requirements of 10CPR55.25 and 10CFR50.74(c).
Once the individual returns from disability leave, he will be further evaluated by an examining physician to determine if this disability can be accommodated in accordance with the guidance provided in Sectian 5.1 of standard ANSI /ANS 3.4-1983.
Depending on the results of this evaluation, the-individual's license may be terminated or a conditional license may be requested.
The NRC will be notified of the results of this decision in accordance with the applicable requirements.
