COMBINED INSPECTION 50-272/82-12 AND 50-311/82-13 SALEM GENERATING STATION UNITS NO. 1 AND 2 MAY 12 THRU JUNE 8, 1982 The following is our response to the violations identified during subject inspection.

Items of Violation Item A-1 Technical Specification 4.6.1.1.a requires that containment integrity be demonstrated every 31 days by verifying all pene-trations required to be closed'during accident conditions are closed by valves, blind flanges, or deactivated automatic valves secured in their positions.

Certain specific exceptions are made in the Technical Specifications.

Contrary to the above:

On May 22, 1982, the following 3/4" vent and drain valves, which constitute containment penetration isolation points and which are not exempt, were not included in the 31 day verification; 2SJ332, 2SJ333, 2SJ338, 2SJ339, 2SJ340, and 2SJ341.

In addition, valves 2SJ332 and 2SJ333 were not secured in their position.

All valves were closed.

The omission of the listed valves from surveillance procedures SP (0) 4. 6.1.lAl and SP (0) 4. 6.1.1. A2 was due to an oversight.

A preliminary review of surveillance procedures SP(0)4.6.1.lAl and SP (0) 4.6.1.lA2 has been completed.

A second and final review of these surveillance procedures is in progress.

3.

All systems that communicate with the containment are being checked to verify that all required valves are included in the surveillance procedures.

We will be in full compliance by June 30, 1982.

Item A-2 Technical Specifications 6.8.1 and 6.8.2 require that written procedures be established for the activities listed in Regulatory Guide 1.33, Appendix A, and further require that such procedures be reviewed by the Station Operations Review Committee (SORC) and approved by the Station Manager.

Contrary to the above:

The following Operations Directives Manual (ODM) procedures in use on June 1, 1982 and within the scope of Regulatory Guide 1.33 were not reviewed by the Station Manager:

-- OD-3, Large Motor Starting Criteria

-- OD-9, Dropped Control Rods and Power Mismatch Circuit

-- OD-12, Technical Specifications Interpretations

-- OD-15, Use of Operations Department Procedures

-- OD-18, Senior Shift Supervisor and Shift Supervisor Logs

-- OD-20, Shift Turnover Responsibilities

-- OD-37, Use of Safety Tags to Comply With Section 4.5.2 of the Technical Specifications

-- OD-38, Diesel Generator Valid Start Criteria Reply to Item A-2 1.

The need to have these documents reviewed by SORC was overlooked by the Operations Department.

Administrative Directive 13, (Independent Review of Operations Department Documents) has been revised to incorporate the listed documents.

Operations Directives 3, 9, 12, 15, 18, 37 and 38 were reviewed and approved by Quality Assurance and SORC on June 2, 1982 and reissued.

Operations Directive 20 was reviewed by Quality Assurance and approved by SORC on June 16, 1982.

We are now in full compliance.
