NRC INSPECTION 50-272/82-14 SALEM GENERATING STATION UNIT NO. 1 JUNE 9 THRU JULY 6, 1982 The following is our response to the notice of violation.
ITEM OF VIOLATION A.1 Technical Specification 6.8.1 and Administrative Procedure l
15, Safety Tagging Program, require that current valve j
status be entered in the Tagging Request and Inquiry System (TRIS) Computer Data Base and that an independent veri-fication be conducted when returning certain systems, including Auxiliary Feedwater, to service.
Contrary to the above:
On June 28, 1982, Valves 12AF10 and 12AF86 were recorded with a status of " Locked Open" as of June 21, 1982.
The valves were in fact open, but unlocked.
Reply to Item of Violation On June 28, 1982, at 1020 hrs., the NRC Resident Inspector reported to the Shift Supervisor that he had found that Valves 12AF10 and 12AF86 were not properly locked open as required by Surveillance Procedure SP (O) 4. 7.1. 2. a (4 ).
The valves, were, in fact, open, but the chains were only locked on the valve yokes and did not prevent operation of the valve handwheels.
The Shift Supervisor immediately dispatched an operator to properly lock the valves.
At 1030 hrs., the Shift Technical Advisor verified that the valves were locked in the open position.

Nuclear Regulatory Commission 8/4/82 Upon investigation, it was determined that the valves had last been operated on June 21, 1982, for a maintenance tag-out.
Following completion of the maintenance, the valves were opened and the chains were improperly installed.
Verification of the valve positions was performed in accordance with Administrative Procedure AP-15.
It is suspected that the personnel performing the valve lineup and verifications checked that the valves were in proper position and that the chains were in place and locked, but failed to notice the improper chain installat o.n.
Action taken to prevent recurrence All operating personnel concerned with the verification of locked valves will be retrained to insure they are aware of the reason for locking valves in position and the proper items to check when verifying a locked valve.
The training will be completed by October 1, 1982.
