Dear Sir:

The PULSTAR reactor facility's Physical Security Plan requires all facility dead-bolt keys, issued and unissued, to be inventoried semi-annually by the Reactor Operations l Manager (ROM) . The surveillance due by November 30,1993 was not performed until January 4,1994 when, at that time, the year-end PULSTAR surveillance verification

! revealed the discrepancy. The surveillance was completed thirty-five days over the semi-annual interval. This constitutes an observed inadequacy in the implementation of administrative controls and requires notification as delineated in our facility Technical Specifications Section 6.7.3(d)2, The surveillance was performed on January 4,1994 and the locations of the dead-bolt keys were verified. At the same time, an investigation was performed to identify the cause l of the problem and initiate corrective actions.

The cause of the inadequacy was an incorrect entry of the last surveillance date in the PULSTAR master surveillance system. In addition, the back-up surveillance board has 2

i historically excluded surveillance requirements associated' with the Physical Security Plan.  !

These two occurrences led to missing the surveillance interval. ,

i The PULSTAR Operations staff has corrected the date entry in _the master i surveillance system and added the security plan surveillance requirements to the surveillance board. These combined corrective actions will greatly reduce the probability of the problem-re-occurring.
