Notice of Violation 93-16-01 Technical Specification 6.8.1.c requires that written procedures be established, implemented, and a
maintained covering the applicable procedures recommended in Appendix 'A' of Regulatory Guide j
1.33, Revision 2. Regulatory Guide 1.33 recommends that administrative procedures for equipment control (locking and tagging) be established. Administrative Procedure 01-S-06-1, 1
Protective Tagging System, step 6.3.2.a requires the individual performing tag installation shall 2
". follow any special instructions as written in the Special Instructions Block."

Contrary to the above, on October 07,1993, operators hanging clearance 933749 and 933750 failed to adequately verify inverter 1Y96 was being supplied from its normal power supply, as j
required in the clearance's special instructions, before opening the alternate supply breaker.

The Reason for the Violation,if Admitted On October 07,1993 a non-licensed operator was realigning equipment associated with j
the uninterruptible power supply system. The equipment clearance had been generated to support the calibration ofinverter alternate source transformer indications. Special instructions had been incorporated into the clearance to direct proper equipment manipulations. The Special Instructions required personnel to verify the inverter was being supplied by the normal power source prior to opening the inverters' alternate supply breaker. The operator observed the inverter barrel switch position but did not verify light indications prior to performing the task. The barrel switch does not change position when an automatic transfer to the alternate source occurs. Indicating lights which are on the j
inverter reveal which power source is in service.
The opening of the above breaker deenergized the associated distribution panel and
]
resulted in the actuation of several ESF systems. The affected systems were restored to their appropriate mode of operations.
8 A subsequent investigation revealed that the cause of the incident was due to inadequate

work practices. Self-checking was not adequately applied while verifying tbc inverter j
status. Operators performing the task did not consider all available information and indications while verifying the power source of the inverter.

Corrective Steps Which llave Heen Taken and Results Achieved Personnelinvolved were counseled. Operations personnel were briefed on the event. A Standing Order was issued to require that an information tag be placed on the barrel
- switch when the alternate power supply was supplying the load. Signs were also placed on the alternate power source supply breakers. These signs give appropriate details for inverter verification which would be required prior to the opening of the breakers l

Corrective Steps to be Taken to Preclude Further Violations
